BOARD AWARDS RECOGNITION
Do you qualify for an Award?
Please review the criteria listed on the “Recognition Check-list” page to determine qualification for a Halton award. An award application can be made for an individual staff member, an individual student, a group of students, or on behalf of a class or school team (staff and students).
Awards are usually in recognition of an accomplishment at the Board, regional, provincial, national or international level. It is important to note this distinction in the awards criteria.
How do you apply for an Award?
Print/copy the criteria check-list and check off the completed criteria for the specific award for which you are nominating.
Also attach a page outlining the following information:
· Who is involved in the achievement and should receive recognition/award?
(Please list students and staff with correct spelling of names).
· IMPORTANT: What was the achievement?
(Please briefly describe the achievement, stating, if appropriate, background of the award/competition, and if other recognition was awarded. Additional information, as appropriate, is welcomed.)
· Who is nominating this person for the award and/or the primary contact person?
(Please clearly print name, school phone number, and email.)
· Who will be in attendance at the Board meeting when the award is presented?
(It is incumbent on the nominator – when confirmation of the award has been received – to invite school administrators and/or other staff/guests to the ceremony)
What are the timelines for applications? When is the presentation?
Award applications should be directed to Anne Fraser in the Director’s Office THREE WEEKS (or earlier) prior to the date of the Board awards presentation.
Applications will be reviewed. Anne Fraser will contact the nominator of the award to provide confirmation of the date, time and location, as well as other information regarding the format of the evening.
Following this confirmation, the nominator informs the award winner, and is responsible for coordinating the attendance of special guests, and writing the introductory script (which he/she will present at the awards night).
Questions relating to the awards/recognition process can be directed to Anne Fraser in the Director’s Office.
