Request a Transcript
Secondary school offices are currently closed. If you’re a current or former Halton District School Board (HDSB) student, you can request your official Ontario Student Transcript (OST) through the School Operations Department by completing the Request for Transcript Form.
Applying to Ontario Colleges or Universities?
- You must be a former HDSB student.
- You’ll need to pay a transcript fee online.
- You only need to request one transcript. It will be forwarded to all your post-secondary choices by the application centre.

Still in secondary school? You do not need to request a transcript - your school will send it directly to OCAS or OUAC for you.
Transcript Fees
- First two copies: $20.00
- Each additional copy (ordered at the same time): $5.00
Requesting a Transcript for Other Reasons
- Contact the last HDSB secondary school you attended.
- Complete and submit the Request for Transcript Form.
- Include a cheque or money order (non-refundable).
- Bring valid photo ID when picking up your transcript.
Mail-in Requests
Mail the completed form, payment, and a photocopy of your ID directly to the last HDSB secondary school you attended.
