To obtain a transcript, contact the last secondary school attended. Requests for transcripts should be made to the Secondary School Guidance Secretary at the last high school attended by completing the Request for Transcript form.
July 15 - August 20
To obtain a transcript, requests should be made to the School Operations Department by completing the online link below which is the request/payment form https://hdsb.schoolcashonline.com/Fee/Details/66548/181/False/True.
Effective after August 21
Secondary School Guidance Secretaries will administer requests for transcripts outside of the noted time frame (July 15-August 20) and in alignment with secondary school offices reopening. To obtain a transcript complete the Request for Transcript form
The Ontario Student Transcript Request (OST) is the official record of a student’s academic achievement at the secondary school level in Ontario.
Transcripts are required for post-secondary institutions and other reasons. If the last secondary school you attended was within the Halton District School Board and you need your student transcript, please follow the directions listed here.
Obtaining Transcripts for Entry to Ontario Colleges and Universities
You need to:
be a past student of the Halton District School Board
pay the transcript fee through the online application process
Note: you just need to request one transcript. Application centres will forward your transcript to your post-secondary choices.
If you graduated more than one year ago and you plan to attend college or university use the following links to order your transcript.
Note: if you are still attending secondary school and are applying to an Ontario college or university you don’t need to request a transcript. The transcript will be forwarded to the college centre (OCAS) or the university centre (OUAC) for you by your school’s Guidance/Student Services department.

Requesting Transcripts for Other Reasons
request your transcript from the last secondary school you attended
complete and submit the
Request for Transcript Form with the non-refundable transcript fee (cash or money order) back to school
Proof of identification is needed when picking up transcript
Mailing in Request for Transcripts
Mail the Request for Transcript Form with the non-refundable transcript fee (cash or money order) and a photocopied proof of ID directly to the last school you attended.
